Rarou does not use the Ming medicine (likely alluding to some sort of opium) which nullifies all pain (later in the movie we see a Ming tortured for a full day without feeling anything at all).
Fuugo: Is the reason you won”t use our medicine that you don”t believe it works.
Rarou: The question is different. The medicine you”re using now at least has some effect. . .

Rarou vs Shogun Itadori (Round 1): Rarou contends with Shogun Itadori. Itadori muses he”s the best he”s fought since Nanashi.
Nanashi vs Rarou (Round 2): Nanashi contends and defeats Rarou by the skin of his teeth (more of an “any given Sunday” victory, as he was saved by emerald).
